Palm oil is the largest business segment (80%) for PT Perkebunan Nusantara IV. PTPN IV has 30 business units that manage oil palm estate that has spread in 9 districts, namely Langkat district, Deli Serdang, Serdang Bedagai, Simalungun, Asahan, Labuhan Batu, Padang Lawas, Batu Bara and Mandailing Natal.

In the treatment process, PTPN IV has 16 palm oil mills Unit (PKS) with a total capacity of 615 tonnes of fresh fruit bunches (FFB) per hour.
